Question: Hello, I am a 35-year old male.
In the past I have occasionally had what I thought were minor abrasions on the head of my penis. These took the form of small red dots/patches, but were never a concern.
About 2-months ago more dots appeared and started to look more like a rash but they were still mostly small (<1mm). They appeared fairly uniformly spread over the head of my penis with perhaps some clustering. The red dots were not sore or itchy. They were also not raised and when touched with my finger they did not feel noticeably different to normal skin. It is not sore to pee nor is there any discharge from my urethra.
A couple of months ago I did once have a white build-up that looked like smegma under my foreskin, however, since then I have been retracting my foreskin and washing the head of my penis with warm water in the shower (no soap).
I thought I might have balanitis (yeast infection) so I applied Canesten cream twice a day for 7 days. After 3-4 days the rash started to clear up and by day 7 it had apparently gone so I stopped using the cream. However, 5-7 days after stopping the cream the rash started to come back so I used the Canesten cream again twice a day but this time for 3 weeks. The rash disappeared after the first week of use and remained gone whilst I was applying the cream. 7-8 days after finishing using the cream I can see the rash starting to come back the same as before.
It may also be useful to know that my foreskin has always been tight and difficult to retract when my penis is erect (fine otherwise), and that on other parts of my body I do occasionally have dry skin.
Do you have any advice as to what this may be and further treatment options?

Based on the fact that you have posted you seem to have been developing genital fungal infection secondary to Partial Phimosis .
I would suggest you you to consult qualified Urologist for clinical examination and get following basic test to establish diagnosis .
1) Microscopic examination of scrapping from glans penis to confirm presence of fungus .
2) Routine urine analysis.
He would be the best person to decide need for surgery of Circumcision based on clinical finding as regards tightness of Phimosis .
In the mean time continue using steroid cream that you are using .
