Question: Hello Doctor,
We had first baby 3.4 weeks backs, we have had an unprotected intercourse today. Would appreciate if you suggest a safe contraceptive medicine as she is breast feeding.

During breast feeding, the chances of conception are low during the first three months after a childbirth. For contraception, it is best to get an intrauterine device ( Cu-T/ Multiload) inserted in the uterus as it gives a 99% protection from pregnancy and does not interfere with the breast feeding.
Other options are progesterone only pill- needs to be taken daily or depot provera injection- given every 3 months - But these both can cause an irregular periods.
The last option is the use of barrier method like condom or spermicidal gel like Today- these have a low protection rate. The failure rate is as high as 20-30%.

Thanks for writing again.
I-pill is a high dose hormonal pill that will affect the breast feed, will cause irregular bleeding and is not a fool proof method of contraception. Its efficacy decreases considerably if taken after 24 hours of unprotected sex.
