What Causes Mild Cold, Fever And Burning Sensation At The Anus Post Defecation?

Question: Hello Doctor,

This is regarding pain while passing stool. Today I got very painful stool, it took almost 1.5 hours for me to pass the stool. Also, at one instance it became so painful to pass the stool that I have to try to cut the portion of stool (which was outside bowel) through hand using a tissue paper. Also, as the remaining portion of the stool went back into bowel, I have to go back to Kitchen and drink around 750 ml of water.
However, once the stool was passed in around 1.5 hours with intense pressure, I started feeling burning sensation in that area probably because of the cuts. Also, I started feeling little cold and feverish. I checked the fever after 5 hours of this activity and found the fever to be 100.1 degree.
I am worried if this fever is because of any infection due to use of hand and tissue paper to cut the stool and as the remaining portion of stool went back into bowel or if it is because of the immense pressure and pain that I faced for around 1.5 hours.

I took Metamucil (Fiber Supplement) and Aleve (Naproxen Sodium) around 5 hours after this activity.

Request for your advice if it looks like a concern and if I have to go to hospital right away. As of now, the only symptoms I have are burning sensation in that area, Fever (100.1 degree) and feeling little cold.

Have gone through your details and i appreciate your concerns.
The term gut microflora or microbiota means hundreds of species of bacteria which lives in our intestinal tractintestinal tract.These bacterias and yeast are responsible for proper digestion and fermentation of food. Hundereds of species of bacteria and yeast re found in colon of human intestine.In the same way thousands of these species florishes in our gut these bacteria play a vital role in digestion and absorption of food.Even minor disturbances in gut microflora can lead to significant increase in colonies of gut bad gut bacteria this lead to incomplete production of fermented food which actscas a source of food for good bacterias and fungus which may cause infection and fever(fever is hust a indicator of infection).Predominance of these bacterias may cause fever etc.Fever is just suggestive of infection by bad bacterium.

Small Intestinal Bacterial Overgrowth can be a reason of severe flatulence .
Small intestinal bacterial overgrowth (SIBO) refers to a condition in which abnormally large numbers of bacteria are present in the small intestine, and the types of bacteria in the small intestine resemble more the bacteria of the colon than the small intestines causing improper and disturbed food digestion.


Tests suggested-
Small bowel imaging and a gastric-emptying scan.

suggestions-Root cause is constipation.
Take laxative lactulose 10 ml twice daily.
Take paracetamol for fever.
Take two tea spoon (or one big spoon) isphagula husk(stop metamucil) with 200 to 300 mL water
- increase warm water( atleast 8 glass/day) intake
- do not drink water during and after meals
- eat small meals (rich in fiber)
- eat plentiful "fresh oranges" and "pear "
- avoid COLD milk and and AVOID junk food(NO burger , pizza-even if its veg.) altogether
- mix few drops of "lemon juice" in warm water. Drink on empty stomach, early in the morning, to aid in bowel movement
- avoid beverages / alcohol/ soft drinks
- do some exercise (push-ups / squats etc) early in the morning
- avoid carbonated drinks
- take 2-3 spoon edible virgin olive oil in the morning.
You would be fine in a week.
-Consume high fiber foods as a part of high biber diet. Grain products: (6-11 servings of grain product in your diet is required daily) whole grain breads, whole wheat, whole grain mixed grain, cracked wheat, buns, muffins. Brown ricewhole wheat pasta or brown rice pastacereals made from whole grain oatmeal ground flax, wheat germ or wheat XXXXXXX in baked goods vegetables: (3-5 servings daily) dark green leafy vegetables contain heigest amounts of fiber e. G. Spinach, broccoli, green peas, green beans, turnip greens. Fruits: (2-3 serving of fruits) apples. Oranges, pears, peaches. Berries (strawberries, raspberries, blueberries, and blackberries) dried fruits (dates, prunes apricots legumes, nuts and seeds
Black bean, green beans, peas, kidney bean, lentils, peanut, soy beanWalnuts, peanuts, cashews, sunflower seeds
High fiber food relieves the constipation which ultimately reduces the abdominal and colonic pressure

Hello Doctor,

Thanks for the advice.
As per the advice it looks like it is not a serious problem and can be cured within a week. Please correct me if I am wrong.
Also, just wanted to check if I can continue Metamucil instead of isphagula husk as it is difficult to get isphagula husk in my locality. I came to USA for a short period and it is a suburb area which doesn't have much options for husk. I am currently taking 1 teaspoon of Metamucil powder with 200-300 ml of water twice a day.
Also, I am having a lot of pain in the bowel area (Around outer area near to the passage from where stool passes), probably due to the cuts. Can I apply some cream on it or should I allow it to heal on its own?

Also, Just thought to add another medicine I started taking yesterday night - Stool Softener (Dulcolax - Docusate Sodium) once a day due to the fear of again getting hard stool. Please let me know if I can continue this instead of laxative lactulose. Asking so as I am not sure if I will get Laxative lactulose over the counter in USA and not sure of the name by which it will be available.

Yes you can take dulcolex 2 tablet before going to bed at night.
Lactulose is over the counter.
Oncrease dose of metamucil 2 tsf twice daily.
Apply preperation H locally once daily(its otc).
