Question: Hello Doctor,
I am Male, 27 years old, Married.
I was diagnosed with Enterococcus faecalis bacterial infection in my semen a month back via semen analysis with reading as "plenty". The Urologist gave me Ampicilin 1000mg, three times per day for 7 days. I waited for three weeks of time and again gave semen sample for testing and it came positive for the same bacteria with reading as "plenty".

Now the doctor has decided to give me Ampicilin injections each day. But he did not mention as how long I should be taking the shots.

Question:
1. How long should the injections be taken?
2. Are there severe side effects due to these injections?

Usually ampicillin is given for one to two weeks depending on the response to treatment. Response to treatment is evaluated by decrease in symptoms and preferably a repeat culture.

Ampicillin is generally a well tolerated medicine and may cause side effects in rare occasions. it may cause gastrointestinal disturbances like diarrhea, generalized rashes, bacterial resistance and super infections and anaphylaxis reactions. That being said since you already took one week of ampicillin I don't think you will develop the above mentioned side effects.

thank you for your follow up question. in my opinion it would be the same effect if the ampicillin is given is oral or intravenous. intravenous antibiotics would react the therapeutic dose faster than oral but after 2-3 doses they would have the same concentration in the body.

if I was your clinician I would change the antibiotic to a different antibiotic based on the culture. there are also reports of intraprostatic antibiotic injections because the prostate is very hard to penetrate with systemic antibiotics.

Thanks for your response. My culture test showed that the bacteria is sensitive to Vancomycin and Ampicillin.
My final question is, are these bacterial infections bound to cure at some point of time or should I be relying on medications throughout my life?
My wife and myself are planning for a kid and this seems to be a great set back. Are there means to get pregnant if the cure is not attained?
Hope I find a cure sometime on this. Quite frustrating.

It seems to me that since there is heavy growth on the culture then treatment is much needed with antibiotics. With proper antibiotics this infection can be treated and you can be rid of this bacteria. definitely this infection can make your semen suboptimal for pregnancy but it is not impossible to conceive even with this infection it will all depend on your semen analysis and how it looks like.

this bacteria can be cured with antibiotics and you don't have to suffer all your life. but this bacteria cannot be cured without treatment and you may suffer chronic prostatitis because of this infection.

my advice to you is to seek a second opinion for this infection preferably with a urologist with specialisation on fertility. that way you will be treated for the infection in the same time improve fertility.
