Question: Hello - I have had OCD for the last 30 years. I have been on many SSRI's - I am now on Trintellix which has the least amount of sexual side effects. I have been on Trintellix for approx 6 months and have had no sexual problems. Lately I have had problems holding an erection and climaxing.
My GP switched me from 100 mg Viagra to 5 mg Cialis daily and 20 mg one hour prior to intercourse. It definitely helped getting an erection and keeping the erection. Unfortunately it did not help with the climaxing.
I can have intercourse first but I only climaxed in the past by mutual masturbation.
I have been on the 5 mg Cialis for only one week and yesterday I tried the 20mg one hour before sex and as mentioned did not manage to climax but did have a good strong erection that lasted longer than usual.
Unfortunately even though my free Testosterone level is that of a "20 year old" as told by my GP my libido is still low which may also be contributing factor in not being able to reach climax.
I am hoping that the longer I am on the 5 mg Daily Cialis by reaching climax will improve - am I correct? Since I am limited by my insurance the number of 20 mg tablets is only 4 a month when do you suggest I should try again?
One side note the last few weeks being on Viagra then Cialis when I have a strong erection and trying to climax - instead of climaxing I am having a small amount of urination which I think interferes with my trying to climax. What is your opinion on this? Is there any hope for me to climax?
Because of the OCD going off the Trintellix is not an option.

First of all, your ability to achieve an erection on Viagra without climaxing suggest a potential psychological cause of your condition: Any recent emotional trauma, stress, etc.? If reducing the dose of brintellix is not an option for you, you could try switching to another antidepressant; has your doctor talked to you about this?

Hi That is fine that you are a urologist. First all the SSRI have worse sexual side effects than trintellix. Follow up question. Again the longer I am on cialis the better it will work? Also what is causing me to slightly urinate during mutual masterbation since I have not been able to climax during intercourse for some tome the only way I am able in past is mutual masterbation after intercourse. I am seeing a sexual psychologist. Agiain I wish libido was stronger but it is blocked by the SSRI. Please let me know what is causing the slight urination since it disrupts me climaxing

Hi again,

I do not find it plausible, that Cialis should work better the longer you take it. Instead, I would suggest the opposite, i.e. going off the regular/daily Cialis, and then take 20mg 30 minutes or so before sexual activities. The urinary incontinence may be a side effect to the Trintellix.

Have you considered discussing with your doctor/Psychiatrist, to try Clomipramine (a tricyclic antidepressant) as an alternative to Trintellix? The sexual side effects may be much less severe.
