Question: HI, I AM A 37 YEAR OLD MALE AND FOR THE LAST SEVERAL MONTHS I HAVE KIND OF A QUICK HEADACHE PAIN FOLLOWED BY WHAT I WOULD DESCRIBE AS A FEELING OF DISORIENTATION, DIZZINESS, SOMETIMES HOT FEELING, AND WHAT KIND OF FEELS LIKE A LACK OF BLOOD FLOW OR OXYGEN TO THE BRAIN. IT MOSTLY FEELS AFFECTED ON MY LEFT SIDE. IT USUALLY LASTS ABOUT 30 SECONDS THEN GOES AWAY BUT IT COMES AT JUST RANDOM TIMES. THE LAST 5 DAYS IT HAS BEEN MUCH MORE FREQUENT. I HAD A SINUS CT SCAN DONE A FEW MONTHS AGO AND IT ALL LOOKED GOOD. ANY IDEA OF WHAT MIGHT CAUSE THIS OR WHAT I MIGHT HAVE? THANKS.

I have noted your clinical details. Based on this, the most likely diagnosis is paroxysmal hemicrania (PH). PH is a primary headache disorder, where no abnormalities are found on the brain scan. There is no need to worry, as it is not a serious condition, and it can be treated with simple medicines such as indomethacin capsules.

Even though we expect a normal brain scan, it would be better to get a brain scan done to exclude any problem.

You can take Indomethacin capsules. It that is not available OTC, then, other options include naproxen or paracetamol tablets.

For my patients, I advise Naproxen tablets at a dose of 500 mg twice daily (one in morning and one at night) for about a week to see the response.

I HAVE NAPROXEM NOW FOR ARTHRITIS BUT I ONLY TAKE IT AS NEEDED AND NOT REGULARLY. WOULD YOU SUGGEST MAYBE TAKING THIS TWICE A DAY (IF SO AT THE AME TIME OR SPREAD OUT IN THE DAY TWO TIMES) AND MAYBE DOING IT FOR A WEEK TO SEE IF THINGS GET BETTER? THANKS.
