Does AKT Intake Cause Fluctuations In BP Level?

Question: Hello Doc,

My dad has been recently prescribed AKT 4 (2 month)and now AKT 2 (28 days and ongoing) for TB in lungs.Chest Xray was not very promising but TB Gold test has come positive for TB.He is facing side-effects like increase in SGPT/Sodium/Potassium levels.These things we are able to cope up because of regular tests and good balanced diet. But the concern arises with BP as it keeps on fluctuating and goes till 200/110,although not very often but usually it is somewhere between 150/90-180/100. He has been diagnosed for BP since 2005 and was completely in control with Amlong-10 mg but recently his BP medicine has been changed to following because of fluctuation (Arkamin-100mg-Morning/Evening,Prolomet XL-50 mg-Morning/Evening,Amlopress-10mg-Morning/Evening) but still situation remains the same.Also Every morning post taking pills, he fills diziness and then rise in BP.
My Concern is
**this whole thing started after AKT-4,so i want to know- Is there any link with BP with regard to this medicine.If it is then we will be relaxed till the completion of medicine course.
**Is there any way i can get some test done to know whether he actually had TB in first place or not.
** Can you please guide us like what to do next.like which field of doctor you should target ?

No relation between Anti TB and BP

Hi XXXXXXX thanks for your quetion on HCM and for your detailed history.

Answer to your first question.

There is no relation between Anti Tb Drugs and rise in blood pressure. May be due to stress of having TB, Blood pressure of your father is rising. Otherwise no pharmacological basis for AKT and high blood pressure.

Answer to your second question.

At present moment , it is very difficult to prove that your father had TB at the starting of treatment, because your father had taken 3 months of treatment. So the lesions must have healed by now.
You can go for CT THORAX and if it shows fibrotic lesion than probably your father had TB and now due to AKT lesions are healed and gives fibrotic scar.

Answer to your third quetion.

At this moment I advice you to continue AKT2 for 3 months to complete total 6 months of treatment.
Consult cardiologist for BP control. In my opinion you should stop arkamin and replace it with Angiotensin Receptor Blocker (ARB) and low dose diuretic (thiazide). As ARBs and thiazide with prolomet xl, amlopress will achieve good control over BP.
ARB and thiazide combination is considered as first line therapy for elderly hypertensive. So should be started in your father's case.
Avoid stress and tension. Ask him to be relax and not to worry about TB.
