Could Chest Pain Radiating To The Back, Hoarseness And Breathlessness Be Due To Hypertension When Diagnosed With CHD/CAD And Mild Kidney Disease?

Question: I am diagnosed with Hypertension, CHD/CAD, mild kidney disease S2, Ascending Aortic Aneurysm 4.3mm, Descending Aortic Aneurysm 3.6mm. I also have Chronic Angina accompanied by shortness of breath and pain in chest that radiates to back and neck ( this is sometimes with exertion and sometimes without and subsides with nitrate). I have been having symptoms of tenderness and dull pains in mid chest area that radiates to side and back with coughing, hoarseness and shortness of breath for the past 2 wks. The above symptoms have yet to be diagnosed with multiple test. Could the more recent symptoms be symptomatic of the Aneurysm? I also discovered my hypertension over 40 years ago but had gone untreated for 10 years or more. Could all of my other conditions be a result of the HBP?

Hello,

The hoarseness and shortness of breath can be because of enlarging aneyursm. However, the size of ascending and descending aneurysm as mentioned by you should still be treated conservatively by good control of blood pressure (a contributing cause in your case). Also, the size mentioned by you wouldn't cause compressive symptoms.

There is a discrepancy between the symptoms you describe and the mentioned size.
A Recent CT scan would be justified to know the exact size of the aorta and the aneurysm and also if the arch is enlarged as it would contribute to hoarseness if a nerve traveling in that area is compressed.

The other probable causes to symptoms are:
Epigastric pain because of reflux gastritis
Angina
Uncontrolled hypertension
Rarely aortic dissection.

Do rule out each individually and discuss the same with your doctor.

Optimal control of BP, preferably below 130 /80mmhg
Antacid regularly.
A symptom-limited stress test to rule out significant angina and treatment for the same.
