Brief Answer:
follow the advises.. maintain a healthy lifestyle..

Detailed Answer:
Dear XXXX, Welcome to HCM,

I've read your query, reviewed all those investigations and understand your concern..

Now going through your reports , it's quite evident that your titre of WIDAL has come down significantly compared to your previous report , which is a sign of recovery(I believe you must've taken appropriate antibiotics for at least 2 weeks)..

So feel relaxed , as you're on the verge of complete recovery from Typhoid..

However, as far as infectivity is concerned, yes typhoid can spread feco-oral route(means S. typhi is passed in the feces and sometimes in the urine of infected people. You can contract the infection if you eat food handled by someone with typhoid fever who hasn't washed carefully after using the toilet. You can also become infected by drinking water contaminated with the bacteria) ..

Also it includes inappropriate handling of vomitings, you must ensure proper disposal in black polythene bag and most importantly 'NO SPILLAGE' outside , and using gloves while disposing, proper thorough hand-wash with antibacterial solutions afterwards and every time even when going for any purposes to the wash-room..

So as you can see the main challenge here maintenance of proper hand hygiene is a must..

Also ensure that the door-handle of the wash-room is cleaned(this are small area which creates big problems), same goes for your water-dispenser and RO line..

Another important thing is to not letting them come near the radius of you while doing those works.. Explain them thoroughly these whole process beforehand, they'll understand for sure..

As for infectivity phase goes, up-to One month(from the date of detection and commencement of antibiotic therapy) is considered significant..

So I hope I've cleared your doubt regarding this..

Let me know if you have any further doubts..

Best Wishes
Kind Regards

Hi Doctor,
Thanks for the detailed explanation on how to avoid infection.
I had a WIDAL done yesterday and the titres XXXXXXX up again.
it was 1:40 and 1:80 previous time. But yesterday WIDAL showed 1:80 and 1:160.
But doctor says WIDAL need not be trusted always. I am having cold and cough since 3 days.and very mild fever. He says, my previous symptoms of very high fever are not there now . So he recommended ascoril, betadine for throat and cough. And asked to used a antibiotic which works for both cold and typhoid- Maclar 500.
Is this ok to assume that WIDAL is wrong? is that drug above also for typhoid?
Please advice on what additional care can be taken to avoid relapse of typhoid if its cured.

Brief Answer:
stop rechecking WIDAL , specially after 3 weeks it's of no use..

Detailed Answer:
Welcome back dear,

Yes your doctor is right.. but you haven't told me what was the antibiotic that you've taken last time ? Also mention the complete dosage , the total duration of your therapy as , if inadequately treated, typhoid has a very high rate of relapse as well as antibiotic resistance and troublesome complications..

Please get back to me with full details so that I can guide you in a better way and able to tell you about the relevance of MACLAR(Clarithromycin)..

Those symptoms , you've mentioned can be due to viral influenza also..however since your doctor has already checked you physically, so he might've been the better person to decide in this regard as quite certainly I don't have that privilege here..

So please have faith in the treatment course and once started , always complete the full course of antibiotic which is most important..

Like I've already advised in my previous answer, hand hygiene, proper washing of food products, maintain healthy surrounding of the water dispenser and RO lines, complete avoidance of outside water and outside salads, ready made food products are the things to do to prevent diseases like Typhoid in the long run..

Take Care
Let Me know how you stay in the due course..

Best Wishes
