Question: Background Information:
40 years of long distance running (marathons). Last marathon Oct 2018. Low resting heart rate for many years.
Episodes of dizziness ~ 10 years ago. Found to be very low levels of vitamin D. No occurrences since taking daily vitamin D (6-10K IU).
Suffer from periodic bouts of insomnia. Diagnosed with mild / moderate apnea via sleep study. Being fitted for CPAP to counter occasional snoring.
Suffered L5/S1 herniate disk in 2013, lost some nerve function in left leg but was able to resume normal activities after 2-3 months of rest and rehab.
Re-herniation in March of 2019. Severe left leg pain, foot numbness. Had micro-discectomy performed by Dr. XXXXXXX Brebach in May of 2019. Relieved primary symptoms but left some residual loss of sensation in left foot.
Current Symptoms:
Noticed slight muscle fasciculation’s in left buttocks and upper leg several months after surgery. Reported to surgeon. Calmed down after several days. Late fall started noticing muscle tightness in left glutes and hamstrings that would resolve after several days. Fasciculation’s have gradually increased in frequency and occur throughout left buttocks and leg.
More recently noticed left leg / foot get tired effecting gate. Was walking 4-6 miles daily. That is now much more difficult. Reported these symptoms and saw surgeon in Early February 2020. X-rays and exams did not reveal any issue. Prescribed Celebrex. Saw family Dr. who told me to stretch more and return in a month.
Most recently notice some muscle fasciculation’s in other parts of my body. Left arm, left chest, left leg. Very slight. Other recent symptoms include increased saliva forcing clearing of throat often. Some Tinnitus in left ear noticed. Very concerned about serious neuro problem.

I have noted your clinical details as well as the details of surgery/medications.

Based on the details, the most likely cause of your symptoms is the pinched nerve in lumbar region due to herniated disc. The disc herniation-related nerve disease is called lumbar radiculopathy. Lumbar radiculopathy can cause all symptoms you described including muscle twitches, weakness, pain, tingling, etc.

I guess your main concern could be a serious disease such as ALS (amyotrophic lateral sclerosis), however, it does not seem so. You could discuss this with your neurologist. If any doubt remains, an EMG (electromyography) study can be done to rule out ALS.

Medications such as gabapentin or pregabalin are used for relief of symptoms (pain, tingling, etc).

Physiotherapy including back extension exercises are also helpful.
