Why Is My BHCG So High With No Fetal Pole And No Heartbeat?

Hi I am by dates 10 weeks + 5, (no sexual intercourse in last 6 weeks so can't be much less) had my first scan 3 days ago, gestational sac 36mm no fatal pole no heartbeat. I am waiting for another scan in a week. I have felt very sick and have checked my BHCG (I am a GP) as was worried about molar pregnancy and it is > 35,000. Scan was TVS so hospital are reassuring me that unlikely molar pregnancy but I am surprised my BHCG can be so high with no fatal pole etc. Is this a missed miscarriage or should I be worried about molar pregnancy?

Hello and welcome to HCM,
A high HCG levels without identification of fetal pole can be due to molar pregnancy, missed abortion, blighted ovum, threatened abortion.
The levels of HCG are very high in molar pregnancy and tend to increase rapidly.
The HCG levels are very high in your case.
Molar pregnancy can be identified on the ultrasound and shows a characteristic snow storm appearance.
The next step is termination of pregnancy.
You need to discuss it with your gynecologist for the most appropriate method of termination at this point of time.
The examination of products of conception can only determine the character of the products- whether molar pregnancy or abortus.
