welcome to Health care magic. 1.Lymph nodal enlargement is seen in the cases of infections / inflammations , long standing systemic disease, and lymph nodal pathology itself. 2.The cough and fever for a long standing also can cause lymph nodal enlargement. 3.In this case first thing i would have done if to rule out presence of tuberculoses, if it comes negative than a lymph nodal biopsy. 4.As before we start treatment for pyrexia of unknown origin ( fever with no identified cause) we need try to find the cause. 5.Suggest you to get an appointment and see your GP and get the needful investigations done, do not delay. 6. One more thing considering his age if there are any systemic causes - please make sure they are in control. Hope it helps you. Wish you a good health. Anything to ask ? do not hesitate. Thank you.
