What Does HLA-B27 Negative And B7 Positive Mean?

I just returned from the MAYO Clinic in Jacksonville and I was seen by a rhumy for a possiable DX of Spondylitis. HLA-B27 test came back negative and states I'm B7 positive. What does all of this mean?? Could I still have Spondylitis with B7 positive and B-27 negative?

Hi,
Thanks for writing in.

There are different types of ankylosing spondylitis. HLA B27 is a marker and need not be present in every patient. Similarly HLA B7 is also a marker and can be seen in ankylosing spondylitis. Therefore you might have a subtype of the condition that is HLA B27 negative and HLA B7 positive. This needs evaluation based on your symptoms. HLA markers are by themselves not specific to any condition but in the presence of symptoms, certain conditions can be better recognized with HLA tests. Please disccus your symptoms with your doctors and this will help in making a diagnosis of HLA B 27 negative and HLA B7 positive ankylosing spondylitis in you.
