Hello. Thanks for writing to us at healthcaremagic.

Acne can relapse after seemingly complete cure. As such there is no one time solution for acne.
The medicine which worked for you contains retinoids (Isotretinoin) and is considered to be the best form of oral treatment for moderate to severe acne. I must assure that weight gain is not a side effect of Isotretinoin.
Since acne patients may experience a relapse, therefore some patients may require a repeat courses of Isotretinoin.
Relapses are more frequent in those who don't take isotretinoin for the recommended duration (usually 6-8 months, twice daily dosing) Or a total cumulative dose of 120-140 mg/kg.
If your dermatologist feels that you require Isotretinoin for your severity of acne, you may safely restart without worrying about weight gain as a side effect of this medicine.
I would suggest that you discuss with your dermatologist if it would be better to take Isotretinoin for the recommended duration Or a total cumulative dose of 120-140 mg/kg to prevent future relapses.
Your dermatologist after proper assessment might also start you on oral antibiotics and topical antiacne medications along with Isotretinoin.
A few sessions of salicylic acid chemical peeling once every 2 weeks, may control your acne activity faster.

Regards
