Suggest Treatment For Swelling In Throat After Treated Lymph Node TB

Dear Doctor- My daughter is 17 Yrs old diagnosed with Lymph Node TB. She had swelling at her Throat at left side. After FNAC test doctor started treatment with AKT4 (TB Drug). After taking AKT4 for 2 months, the swelling has been reduced by about 85%. Dr. changed the medicine and prescribed : R-Cinex 600 Mg with Benadone for 4 months. Now my daughter is taking R-Cinex 1 Tablet daily since last 22 days. Now, she is complaining vomiting tendency as well as stomach pain and gas formation. The doctor had told in spite of some side effects, she has to continue the medicine. Currently my daughter is staying in a residential school away from home. The Doctor treating her had told earlier that, in case of any other health problem she may consult local doctor and take medicine but the doctor to be told about the TB medicine she is currently taking. Now my question is: 1.     I feel AKT4 was effective that is why swelling reduced considerably. Now due to change in medicine, how long will it take the swelling to go completely? 2.     She had been advised to visit local doctor for frequent stomach pain. Whether, she can take medicine prescribed by local doctor along with TB medicine? 3.     After 2 months of medication, Liver Function Test was done and findings were Ok as confirmed by doctor. The test was conducted on 12.05.11.Her Bilirubin (Total-Serum) was 0.3 mg/dL. Is there any scope of Joundice now?

General Surgeon 's  Response
Hi,

Noted the history about your daughter diagnosed and under treatment for tuberculosis of cervical lymph nodes well diagnosed by FNAC, initial 2 months was give AKT 4 and now shifted to R-cinex, started pain in abdomen and vomiting, liver function being normal.
Yes, consult your local Doctor for proper clinical evaluation, examination and have investigations for the pain in abdomen and vomiting.
The commonest cause I can think about the symptoms are tuberculosis of intestines which get scarified with treatment.
Also get an opinion of a General Surgeon trained in intestinal surgery.
CT scan of abdomen with enteroclysis can help a lot.
Upper GI Endoscopy for esophagitis and gastritis and if present additional treatment with a PPI like Omeprazole can help a lot. Other symptomatic treatment can be had with Domperidone.
Review tests of blood for liver functions, kidney function can help.
She has to continue with treatment for tuberculosis as would be advised for further months.
High protein diet, correction of anemia can help a lot for early recovery.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, ENT and General Surgeon
