On 29th March-2007 afternoon (In Office) I was feeling for urine passing problem & headache, next day I had admitted to Peerless Hospital Kolkata under (Dr T K Banerjee-MD-FRCP-FNN) on 30th March-2007 due to urine passing, headache & weakness of both lower/upper limb & after that, Treatment started (Solu-Medrol) 1gm intravenously for 5 days & Medicines (RANTAC, DUPHALAC for 7 days, URIMAX for 7 days, WYSOLONE for 7 days, GABATINE FORTE to continue , NURODAY to continue. Also consulted with Dr S P Sinha (Physiatrist) after regained normal power & discharged on 11th April-2007. 1st Phase Clinical Test/Diagnostics/Investigation 1.MRI of Brain 2.MRI of Cervical Spine 3.MRI of Dorsal Spine 4.CT SCAN of Brain, 4. Radiography of Chest PA 8.VEP (Flash) 9.Blood Test for Cell Count (CSF) & Cell Type (CSF) 10.Plasma Glucose (Random) 11.Protein (CSF) & Sugar (CSF) 12.Serology Prostate Specific Antigen 13.USG (KUB + PROSTTE) 14. Routine Urinalysis 15. HB, TC, DC, ESR & BTC 16. Urea, Cretin’s, Sugar Fasting & Sugar PP 17.PNS (OM) View, SKIAGRAM of Chest PA View & Cervical Spine AP View Etc. 1st Phase Final Diagnosis ADEM 2nd Phase After few days I had been admitted again to Peerless Hospital Kolkata under (Dr T K Banerjee-MD-FRCP-FNN) on 23rd April-2007 due to RIGHT optic nerve problem after that, Treatment started (Solu-Medrol 1gm intravenously for 5 days & Medicines (PANTADOC, DUPHALAC for 7 days, WYSOLONE 20mg for 7 days, GABATOR-M to continue , REJUNEX to continue, NURODAY to continue & all test had been done (All copy attached herewith) & discharged on 30th April-2007. 2nd Phase Clinical Test/Diagnostics/Investigation MRI of Brain & VEP (Pattern Reversal). 2nd Phase Final Diagnosis 1. OPTIC NEURITIS (Right) 2. Follow up case of ? ADEM. 3rd Phase (for check-up purpose only) After few days I had been admitted again to Peerless Hospital Kolkata under (Dr Debasish Chowdhury-MD-FRCP) on 23rd Nov-2007 to 25th Nov-2007 in absence of (Dr T K Banerjee-MD-FRCP-FNN due to mild headache & pain in RIGHT eye (optic nerve problem & treatment started & all test had been done (All copy attached herewith) & discharge on 25th Nov-2007. Medicines are PREGALIN 75, LIBOTRYP & NURODAY to continue. 3rd Phase Clinical Test/Diagnostics/Investigation 1.MRI of Brain 2.VEP (Flash) 3.VEP (Pattern Revrersal) 4.Blood report for HAEMATOLOGY, HB, TC, DC ESR 5.LFT 6. URIC ACID. (All copy attached herewith) 3rd Phase Final Diagnosis 1. ADEM & OPTIC NEURITIS (Right) of Old case. 4th Phase Since Dec’2007 to Mar’2008 I had some minor problem & after few days I had been admitted again to Peerless Hospital under (Dr T K Banerjee-MD-FRCP-FAAN) on 26th March-2009 to 28th March-2009 due to headache & left eye vision (optic nerve) problem & Treatment started (Solu-Medrol 1gm intravenously for 5 days & Medicines & all test had been done (all copy attached herewith) & discharge on 28th March-2009. Medicines are PANTADOC (40mg) ,PREGALIN (75mg), LIBOTRYP & FELICTA-OD to continue. 4th Clinical Test/Diagnostics/Investigation MRI of Brain, VEP (Pattern Reversal), Brain steam Auditory Evoked, OLIGOCLONAL BANDS CSF, Blood Test for Plasma Glucose (Random), Serum LFT, ANTI CCP3, ACA, ANCA, C-ANCA, P-ANCA, ANA. 4th Phase Final Diagnosis 1. OPTIC NEURITIS (Left) 2. Acute MULTIPLE SCLEROSIS 5th Phase As per my Physical problem I had been visited to CMC Vellore & as advice by Dr Mathews Alexander (DM-Neurology) had been admitted again to CMC Vellore on 23rd May-2009 to 30th May-2009 & Medicines are MYCEPT 750 mg for 2 time, PREGALIN (75mg) for 2 time , LIBOTRYP, NEUROBION 1 od & LIVOGEN 1 od. 5th Clinical Test/Diagnostics/Investigation MRI of Brain with DW/contrast/T2 Sagittal screening of Cervicothotacic Spine, EVP, EMG, All Blood test, Urine test, All CSF test, USG & X-Ray had been done in CMC Vellore. 5th Phase Final Diagnosis OPTICO SPINAL DISEASE RELAPSING REMITTING MULTIPLE SCLEROSIS Now I would like to know yours opinion & what I have to do now for better treatment or cure. But now I am not taking any medicines.
