Hi,
Looking to the history it seems that there might be having some problem with coccyx bone and sacral vertebrae like sacralisation of lumbar vertebrae.
Go for x-ray lumbo-sacral region AP and Lateral view.
Consult orthopedic surgeon and get examined.
Ok and take care.
