Hello!

Thank you for asking on HCM!

I would like to explain that a total cholesterol level below 5.2mmol/l is within the normal limits.

Are you referring to your total cholesterol level or LDL cholesterol with the value 3.6.

Only if this is a LDL cholesterol value, you would need treatment with simvastatin.

I recommend performing total cholesterol/HDL cholesterol ratio, which is a more confident indicator of cardio-vascular risk and would help in the treatment decision.

Regarding your symptomatology, I would recommend to closely monitor your blood pressure (a low or high blood pressure can mimic these symptoms) and perform a Doppler ultrasound of your neck vessels to examine better the blood flow and rule in/out any possible stenosis.

Hope to have been helpful!

Best regards,

Dr. Iliri
