Thanks for posting your query.
Your brother is most likely suffering from acute viral hepatitis, it may be hepatitis A or E.
Presence of raised bilirubin along with raised SGPT (very specific for liver disease) suggests the diagnosis.
Very first week of acute viral hepatitis is symptomatic and may require hospitalization.
SGPT in thousands occurs in only 4 conditions: Acute viral hepatitis, drug induced hepatitis, ishcaemic hepatitis and sudden passage of gall stones from common bile duct.
You should consult with internal medicine specialist and should go for thorough check up.
He should also go for ultrasound imaging study for better clarification.
He should get hospitalization if severely ill.
Recovery requires at least two months.
Take care,

Gastroenterologist Dr. Charles S Narasi's  Response
Hello, sunakshi.8731,
Your brother definitelyhas a type of hepatitis. If Type-B antigen is
negative and also C is negative, did the doctor say he may have Type-A
acute hepstitis ? Sometimes, in the early stages HbsAg will come back negative.
In the acute infection, if these other serological markers are positive, that
makes the diagnosis of Acute B Hepatitis. These are the following-
IgM anti-HBc-Positive
With the symptom of stomach ache, I would think ,he probably ate
some contaminated food and has developed Acute Hepatitis.
The Bilirubin level right now is not very high but may go higher before
coming down. If his fever does not come down and he is not able to
keep anything in the stomach, he might need intravenous fluid support
and hospitalization to follow the enzyme and bilirubin levels. When once
they start to come down, then he can be cared for at home. Type A and
also Type B acute hepatitis, majority will recover completely without
permanent damage to the liver. Hope you can discuss these with your doctor.
I wish him well.
