Hi,

Your son is just 3.5 years old hence it is important to try the medical management first. The management for enlarged adenoids with tonsils causing secondary ear problems like glue ear, Eustachian tube dysfunction should be treated by a combination of an antibiotic, anti-inflammatory medicines along with topical that is intranasal as well as oral decongestants.

Antibiotic may be changes after 7 to 10 days of use and continued with another if required. Short courses of oral steroids like Deflazacort as per the weight helps a lot for regression of adenoids. Local treatment of ear if required can be given and even use of Grommets may be needed for middle ear ventilation. Continue the medicines for 3 weeks and have review by clinical evaluation, nasal endoscopy, X-ray or MRI for adenoids and nasal airway passage.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
