Is It Possible For My Wife Who Has PCOS To Get Her Period Without The Follicles ?

Hi Doctor , My wife (age 28) has PCOS and she went for follicular scan on the 15th day of cycle She had taken Letroz from 2nd 6th day In the scan there was no follicles found Is it something to worry for? She has been taking Letroz for past three months but this is the first time she has gone for the scan. We have been married for 5 years. She is not anemic . On taking Letroz, Duphaston and Folinine for three months she got periods in - 28 days, 45 days, 36 days. Is it possible that without the follicles being formed she will get her period? My understanding is that follicles leads to the formation of egg and then it is followed by either period or conception. Is it right?

Without follicle also period comes.
Nature prepares proper bed to welcome fertilized ovum.if ova is not fertilized then prepared endometrium will be pilled of and resulting bleeding.
Due to lack of hormone which is due to absence of fertilization bleeding occurs.

If your wife is not having follicles after treatment with letrozole then she need to be evaluated thoroughly. First investigation will be to know present quantity of follicles in ovaries( Ovarian reserve) by doing blood tests like FSH, Estrogen,day 2 antral follicle count by ultrasound. Other reason could be resistance/failure to letrozole. In case of resistance she may require other drugs for ovulation induction like clomiphene or gonadotropins. Regarding your doubt about getting periods. Formation of egg or fertilization is not necessary to get periods. In your wife's case there seems to be hormonal disturbance where eggs are not forming or if they are forming there is no ovulation. This is the reason why she is put on letrozole. Consult an infertility specialist soon. All the best
