General Surgeon 's  Response
Hi,

Severe coughing for 3 weeks, not responding to cough syrup and consulting 2 Doctors, shortness of breath and so on could be due to one of the following reasons:
Pharyngitis with irritation or GERD.
Bronchitis.
Post-sinusitis with post nasal drip.
Hence I would advise you the following:
Consult another Doctor and insist for investigations:
Tests of blood like CBC, ESR, blood sugar and as required.
X-ray o Chest and sinuses and CT scan if any positive findings.
Further management as per the findings should help you.
Blackheads anywhere in body of a 3 year old boy are almost unknown and particularly in ears. The most probable cause can be anything from dermatitis, repeated insertion of some foreign body by your son as this is common at this age, some black pen marks and so on.
I would advise you the following in such a situation:
Consult an ENT Surgeon so that with endoscope he can know what exactly is the problem.
He will also clean the wax or advise you the way the problem is presented.
If this is a skin problem he may refer your son to a Dermatologist for second opinion.
Do not try anything at home as the skin is tightly adherent to the underlying cartilage and bone and trying something at home will surely complicate the case.
Watch your son for him putting something in ears.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
