Does Persistent Anal Bleeding From Fissure Need Surgery?

I m suffering from anal bleeding with severe pain after defecation. I had contact to my family doctor he diagnosed anal fissure and they suggest sitz bath,xylocaine ointment application &meal with fiber rich items ,but the problem is still persist bleeding continued and a tinny tissue bulging is perceived ,is it necessary to operate .

The fissure which is not responding to a conventional line of measures in terms of medicines, require surgery for sure to recover in the best possible way.

The tissue bulging sounds to be sentinel tag and the same can be excised in the same sitting very conveniently.
