Have Lipodermatosclerosis, Obese, Losing Skin In Foot. On Betamethasone. Explain Situation

Currently I am very confused.
I have been going to a skin doctor for many years now. He has been treating me for Lipodermatosclerosis which is onset by obesity. I am not questioning the diagnoses however the treatment recommended since 2011. I have been using Ratio-Ectosone Regular, Betamethasone Valerate 0.1% putting on the cream and warping my legs just about every night. Initially a nurse came to my home and applied the cream telling me that it had cordazone in it that thins out your skin and something that goes into your blood steam. Stay off your feet. Sorry but this scares me. My Dermatologist laughed and told me not to be afraid to use it. I went to my GP and requested a second opinion on this matter. As I feel he is not listening to me. At the time there was a side problem of losing about three layers of skin from filing at a foot care place. So in addition to having no cart ledge in my knees having the Lipodermatosclerosis and a foot with layers of skin removed. The foot is healed but painful to go to bed. Very strange. I am afraid that the cream is getting in the foot where the skin is thin.
1. If I go for a second opinion should I tell him all this?
2. Do I have good reason to be concerned?
3. How do I deal with the second Doctor?
4. Should I see a chiropodists?
I feel like I am in a catch 21 situation and no one is looking at the big picture here. It is up to me to make the decision here. I do not know.
(Diabetic/ Nuropia (sp) / Sleep apnea and the list goes on?
HOW DO I HANDLE THIS SITUATION?
AM I TAKING THE CORRECT ACTION HERE.
Inersha is setting in and the situation is getting complicated and I am not sure If I am going about this in the right way. Surely there must be another means of treatment

Hi.u should tell the second doctor all the things applied.peeling of skin is mainly due to wrapping of the leg and try not to touch the other areas.whenever peeling of skin occurs on foot there is defnite burning sensation use a moisteriser to cure it
