From the information given, it seems unlikely to be hormonal in nature. I would like to know the results of the blood tests done so far.

From what you've described as to bleeding from the injection site which doesn't stop easily, your grandson needs to be seen by a Hematologist (Blood doctor) and if no cause has been found so far on routine tests, some special tests will be needed to diagnose the problem.
