Hi, Welcome to HealthcareMagic,
For scabies the dose of ivermectin tablet is 0.2 mg/kg orally once, and repeated in 2 weeks.
121 lbs is equal to 55 kgs approx. After calculating it comes to 11 mg.(For 51 to 65 kg: 12 mg orally).Ivermectin is extensively metabolized in the liver and should be used cautiously in patients with hepatic disease.

Tab Ivermectin comes at 12 mg tab and is a adult dose for 51 - 65 kg weight (112 - 145 lbs)

Ivermectin is not FDA-approved for this use.Its use is considered for patients who have failed treatment with or who cannot tolerate FDA-approved topical medications for the treatment of scabies.

It should be taken on an empty stomach with water.
Allergic and inflammatory reactions may occur with ivermectin.

I am very wary about using Ivermectin. Is there any alternative? As I mentioned, I cannot tolerate Permethrin cream. Also, how long after taking Ivermectin on an empty stomach should one wait before eating?

Hi, welcome again,

Ivermectin is a safe drug and i have prescribed this to more than thousand patients without any side effects.

There is no oral alternative.
Topically you can apply crotamiton lotion.

Wait for at least half an hour after taking tab ivermectin for food.
