Embryo fragmentations is seen commonly due to following reasons:

1) Unhealthy or immature oocyte which is fertilized but no proper division.

2) Healthy egg but unhealthy or infected sperm / semen leading to fertilized fragmented embryo.

3) Improper culture conditions in the laboratory where embryos are going.

4) Problem with incubator with temperature or carbon dioxide concentrations leading to fragmented embryos.
