Question: Hi am I am 39 years old female. Last two years I have been on Aten 25 mg once a day. Prothiaden 50 mg once a day for last 3 years for migraine. I have lonazep sos for anxiety.

I see fluctuations in bp that are very high particularly when im emotionally disturbed or overthinking something. Eg today I woke up from an afternoon nap with a 129/107. Within minutes the second reading was 96/68 and the third was 121/65. I also have recently been diagnosed with thick endometrial lining , possible polyp, which needs a d&c. Not sure about whats going on inside me!

Have Aten 25 mg once a day last 2 years for bp
Prothiaden 50 mg once a day for migraine
Lonazep sos for Anxiety.

Had PCOD 15 yrs ago which was cured.
Had hypothyroidism which got overcorected so discontinued medication about 4 yrs ago.

Got a full body health chk done recently and have the following:

Fibroglandular parenchyma for both breasts- Mammogram-BIRADS1


USG whole Abdomen with TVS shows:
Thickened endometrium with a small focal echogenic vascular area- poly
Suggested to have a Hysteroscopy, polypectomy and D/C

My mother was recently daignosed with stage 2 ductile carcinoma. And fathers immediate sister had it, so there is a high probability of cancer im told.

A lot of issues there but my main worry is about the Fluctuation in BP. Is such a heavy variation normal . One day it was 179/117 and next morning was 103/77. Are all these linked to hormonal issues. I must add that I have been under extreme emtional stress last 14-15 years and feel there could be a connection somewhere. Are there in further tests I need for the hormones ?

Yes all three of your problems could be hormone related and get exacerbated by stress.

Regarding your Blood pressure, yes it can fluctuate like this in many patients. Best solution would be to do a Holter Monitoring for 24 hours. This will give a very good idea of your records through the day and night and also tell us whether it peaks at a certain time everyday so that medication can be appropriately given.

Your Mammography should be repeated every two years.

Yes the suggestion to undergo Hysterescopic D & C and polypectomy is correct.
You can revert back for further clarifications---regards

Thanks doc. Im worried abt long term effects of so much medication.

Also is there any other way to check on hormonal imbalance and control it?

I would actually like to withdraw Prothiaden and Lonazep totally and prefer you to take only Aten. I would also get Holter monitoring done, treat the Blood pressure properly and get into a good exercise program.

Regarding further tests to find out cause of hormonal imbalance, I would not advise it. Further tests may mean further medication. A good lifestyle is the best medicine. Regards
