What Does This Echo Report Indicate?

Question: Hi, I am taking treatment for depression since 1999. This May, during routine checkup, I had some heart problem diagnized. I have attached the echocardiogram report in this question.

My Cardialogist prescribed lacilactone 25 mg, telma 20 mg, clopilet 75 mg and concor cor 2.5 mg one capsule daily until last week. Last week, my blood pressure was 90/60 and therefore, my Cardiologist prescribed clopilet 75 mg and razulor-f one capsule for five days last Saturday. I have a review with my Cardiologist this Thursday.

The Internet projects a scary picture stating that heart problems cannot be cured but can only be controlled. My Cardiologist is confident that I will improve in my heart parameters after six months of treatment. Is my condition treatable with medicines?

Dear Sir
The ECHO report is suggestive of low pumping of your heart (35%) with normal heart size. This is usually associated with a condition called MYOCARDITIS. Some of these patients go on to develop heart failure and enlargement of heart over a period of time and some of these recover completely. A proportion of patients will recover partly.
However, with this report you should have some symptoms. These include shortness of breath, chest heaviness and easy fatiguability. Since heart is not enlarge as yet, this process is not very old.
One more important thing is that at times some ECG changes produce echocardiographic abnormalities which are not well detected by inexperienced eyes. Such abnormalities cause changes akin to low cardiac pumping but actually they do not cause so. One such abnormality is left bundle branch block.
I would request you to upload your ECG graph so that I can throw more light on it. Please also tell me if you have any symptoms especially on exercise / exertion/ running/ using stairs.

Dear Sir
1. Your ECG does not have Left Bundle Branch Block, hence it is not an artifact due to LBBB.
2. Still, I am not very much convinced with your level of activity on which you get symptoms, the ejection fraction of 35% and the normal sized left ventricle. I feel something is amiss.
I would like to get a repeat ECHO from a reputed good institute (like a Medical college) in such cases in my patients. That will clarify the things better.

both reports are similar.

Dear Sir
1. I saw both the reports. They are more or less same with not much difference.
2. You are receiving standard medicines. Although I do not use rozular F in my patients and I prefer aspirin over clopilet in most patients.
3. I get thyroid profile, serum calcium, serum iron and serum phosphate in all my patients with unexplained low pumping. Hope these have been done in your case.
4. I am still not much convinced about the cause of low pumping with normal heart size in your case. So be under the observation of a good cardiologist.
5. Prognostication in a single case can not be done. However as I told you before these patients have mixed fate.
