Question: There is a lobulated T2 hyperintense lesion with stippled enhancement within the left intraconal orbit, with an internal fluid level. Foci of T2 hypointensity may represent phleboliths. The focus measures 14 x 15 mm, and there is mild downward mass
effect on the left optic nerve.

From What you have written, it appears to be a Space Occupying Lesion within your Left Orbit, just behind your Left Eye, pushing on the nerve which carries signals from your eye to the brain. You have not asked any Specific Question.

I wanted to know treatnent options. Thus is result for my 6 year old daughter who has precicioys puberty and blurry visuon in left eye

If Vision is Affected, IMMEDIATE Surgery may be needed

Dear Madam

Welcome Back

Regarding the Intra Orbital Space Occupying Lesion
(a)
If Patient has Double Vision, or If there are disturbances in Vision in form of field defect or visual acuity, or if there is pain in Left Eye IMMEDIATE SURGERY is needed. The Surgery can be done either by Craniotomy (Frontal Craniotomy and Approaching through Roof of Orbit) or Endoscopically or through Orbital Approaches
(b)
If there is no double vision and if there is no disturbances in vision and if there is no pain, we can try conservative methods for three weeks and check whether the lesion reduces in size

In your case, there is Blurry Vision in Left Eye. So Immediate Surgery may be the best option
