What Does My Ultrasound Scan Report Indicate?

Question: I have my first ultra sound scan yesterday, at around 20dpo.
There is a small sac at 4 mm via vaginal scan.
However, I did two follow-up blood test and the doubling rates has significantly decrease from 25 hours to 53 hours then 84 hours, details as follows:
16dpo beta-hcg 767.2 progesterone 179.3 nmol/L
20dpo beta-hcg 2424 progesterone 152 nmol/L
21 dpo beta-hcg 2928 progeterone 102 nmol/L
Does it indicate miscarriage? I have no bleeding or bloating, but my pregnancy symptoms go as well, no hungry even with a light breakfast, no nausea or anything related.

Though your serial serum beta-hCG titres do not show encouraging findings, a confirmation of a missed pregnancy can be had only after a repeat trans-vaginal sonogram at 30 DPO. If a viable sac is not found at this stage, it can be concluded to be a missed pregnancy.

It is not necessary that you should have all the symptoms of pregnancy; many people don't. If you find the symptoms which were present earlier disappearing now, it is significant.

In the current scenario, I would ask you to see a specialist immediately to see if you need any hormone supplements to help the progress of the pregnancy. A clinical evaluation helps to plan further management.

Thanks for your writing, something I really need at this stage. Feel like I have somebody knowledgable and caring stand by my side and don't mind my long boring talk.
Everyone thinks I'm too much on taking blood tests but I just can't help myself.
Please take time to look at the details of my blood test from the very beginning:
9dpo: beta-hcg 6.06
13dpo: beta-hcg 95.38 and progesterone 155.5 nmol/L
15dpo: beta-hcg 399.4 and progesterone 137.4 nmol/L
16dpo: beta-hcg 767.2 and progesterone 179.3 nmol/L
20dpo: beta-hcg 2424 and progesterone 152 nmol/L
21dpo: beta-hcg 2928 and progesterone 108 nmol/L

It's natural pregnancy and even before the blood test, I feel like as if I carry twins. The doubling rate of hcg is so supportive, even it's not a diagnostic. But somehow I got the feeling. To add more information, during my early stage, I have backpain and constant cramps which are still there. They are just mild, not something serious.
This is my second pregnancy after the success first one. On my 1st pregnancy, I had no pregnancy symptons except for the exterme nausea at around week 6 (lasting for 2 days).
I want to be hopeful, but not too hopeful.

I just wonder the sudden slowing down beta hcg doubling rate is an indication of vanishing twins, and in this case the best scenario may be there's still one healthy baby?

I just talk to my doctor and she prescribes me with:
1. duphaston 10mg x 30v taken by oral 3 pills per days
2. utrogestal 200mg x 20v taken by vaginal placement 2 times per days

She's on her trip so I will come and see her on Wednesday for further action.

It is good to hear that you reacted very quickly and do continue with the progesterones until you see your doctor. Sometimes, hormone deficiencies early in the pregnancy can lead to such a picture. Please remember that once you start taking external progesterones, the progesterone content of the serum tests goes up automatically.

Coming back to your serum titres, the early ones are suggestive of good progress though the basic titre values are on the lower side of the range. You should repeat the titres every 2-3 days and not everyday. It is too early to detect a vanishing twin at this stage of the pregnancy. In fact, a clear evidence of a twin gestation can be had at 3 weeks post ovulation along with viability indications like fetal cardiac activity.

Please try not to stress out so much as stress itself is harmful to a pregnancy. Please get a proper evaluation with your consultant as early as possible. Take proper rest, avoid strenuous activity and sexual intercourse.

Thanks for writing again.

Headaches can happen as a side effect of hormone treatment. If you can manage with simple measures or at the most, with a tablet of paracetamol, there is no need to worry. If you find repeated headaches which are intolerable, you will have to either change or stop the medication. Usually, the headaches stop once your body gets adjusted to the new hormone status. Please hope for the best.

Yes. You are totally right. My headache's gone away after an hour.
Then come back the cramp and backpain.
I'm an actuary, and actually being busy in the meeting and worksheet make my mind and heart at ease. Should I continue going to work instead of staying at home worrying? My job involves thinking and staying at the desk and meeting room for the whole day, so it should be fine, right?

It is very good to see your spirit and kudos for that. The cramps can occur even with a normal pregnancy as the fallopian tubes propel the fertilized ovum into the uterus and also as the supporting ligaments of the uterus stretch to help the enlarging uterus, with a dragging pain. Since you have had pregnancy earlier, you can understand how it feels like.

You can get back to your work if there is not much stress and strain involved but please make sure that you avoid prolonged sitting which causes rise in pelvic pressure that is not favorable for an early pregnancy. Take a break in between and allow yourself to lie down preferably with the foot end elevated to relieve pelvic pressure. Do not climb the stairs or walk for lengthy distances.
