I am dr.kakkar (dermatologist). I have gone through your query and I have noted your concern.

TPHA is a specific test for syphilis.
A positive TPHA without any history of genital ulcer or rashes on body means that you have Latent infection with Treponema pallidum (the causative organism for Syphilis). If there is a no documented history of treatment for syphilis then I would suggest that you be treated with weekly Inj. Benzathine Penicillin G 2.4 MU, I/M, for 3 weeks. Prior to that a VDRL titres would serve as a baseline to monitor response to treatment.
Your wife (sexual partner) should also be screened for syphilis i.e with both TPHA and VDRL tests should be done and accordingly she should also be treated with weekly Inj. Benzathine Penicillin G 2.4MU, I/M, for 3 weeks, specially if she is pregnant.

Hi.

Both you and your wife should anyways be treated for latent stage syphilis, as suggested in my earlier reply.
Treatment is safe in pregnancy and prevents transplacental transmission of syphilis. The risk of transplacental transmission is anyways very low during latent syphilis as compared to primary and secondary stages of syphilis.
Before treatment she should also be tested for TPHA and you should get your VDRL test.
Antenatal fetal ultrasound should be performed to look for any signs of congenital syphilis.
