What Does My MRI Scan Test Report Indicate?

Question: I have had a private MRI done of my seventeen year old daughter's right ankle. You can download the MRI from here:

She has had considerable pain (9) over the 5th TMT joint. There are several black spots near the 5th TMT joint. She did have this pain for three years (level 3 or 4) but it stopped a year ago and started again two months ago. RICE treatment has reduced this to a sore feeling. There are several black spots near the 5th TMT joint.

She now does three calf raises a day which cause some pain (She was doing 50). But walking is now unpainful for the most part.

The MRI report summary states:

Multiple foci of marrow signal change within the visualised tibia, ankle and midfoot. - end of MRI report summary

The black spot on the tibia was identified 6 years ago as a cyst and not a problem. However this black spot is quite different to the black spots on the ankle. They have sharp lines and some appear hexagonal.

Please note none of the doctors at her surgery can read an MRI and neither can the radiologist. This question needs to be answered by a orthopaedic surgeon who specialises in ankles.

I have uploaded the MRI report. However please ignore references to Fibrous Dysplasia, the radiologist has read her medical history and has Presumed secondary Fibrous Dysplasia. The radiologist doesn't know what Fibrous Dysplasia looks like ( and probably doesn't know what it is). I do know what Fibrous Dysplasia looks like and these black spots are not it!

How was your day?

Hope this message finds you in good health.
I have gone through the complaints and history in particular and understand your concern.

I generally see many cases like this at my clinic,most of them being cases of benign bony lesions and some being malignancies.

The fact that she has this pain,on/ off for quite a period now suggests that the lesion / lesions we are concerned about might have started to affect the concerned area of bone pretty early.

The reason why the initial lesion in tibia was painless and this foot lesion is painful is because this lesion has started affecting the joint. and thats why there is a difference in the texture of the lesion as compared to the other lesions.
Anything that erodes a joint causes pain.

Regarding the diagnosis- As you know, even after MRI, we still havent arrived to a diagnosis yet and still dont know what the lesion is about.

There are many possibilities as far as diagnosis is concerned,mainly-

POSSIBILITY 1- These lesions may only be involving the affected region of the limb- around ankle

POSSIBILITY 2 -these lesions may actually be a localised presentation of a generalised disease- meaning that the main disease might be somewhere else in the body, and the lesions we are talking about might be their satellite distant lesions.

I would suggest you to do-
1 whole body bone scan-at the earliest to find similar lesions else where in the body.
2. bone biopsy/ histopathology of the lesion- this will confirm the diagnosis

3. Tumour markers- certain blood investigations to see whether these are any malignant lesions
