Question: Hello, i am 48 year old I am entering the MRI report and than will ask you a question.
Findings: Metallic susceptibility artifact is visulized at L2-L3 from a prior interbody posterior spinal fusion Graft material is also present in L3-L4 and L5 S1 Intervertabal disc spaces from a prior interbody spinal fusion. Parasagittal imaging demonstrates trace degenerative retrolisthesis of L1 and L2. Static Alignment is maintained throughout remainder of lumbar spine, no fracture dislocation or osseous destructive process is identified.
There is a mild to moderate intervertebral disc space narrowing and desiccation at L1L2
The consu medullar is normal in caliber and position terminating at the superior endplate of L2. The axial images demonstrate the folowing L1 L2 combination of degenrative retrolisthesis and a broad based annular bulge results in mild indention of the ventral thecal sac.
L2L3 at L5 S1 Bilateral decompressive laminectomies have been performed at all 4 levels with resection of intervertebral discs which have been replaced with graft material. No areas of high grade central spinal canal or neural foraminal stenosis are identifid at any these levels. Additionally no locculated fluid collection or pathologic enhancement is identified in the operative bed.

Impression Prior L2-L5 spinal decompression and interbody fusion as discussed above
Mild to Moderate degenerative disc the L1 L2 with degenerative retrolistheis and a mild broad based annular bulge with mild to moderate indentation in the ventral thecal sac Early facet arthrosis

I have been in tremendous pain, low back, groin pain at times right leg. Just started having problem with urinary incontenance and lack of control wettig my self several times a day, by the time I am aware I have to go, its to late. placed on myrbetriq today. Pain has been intense for months now, Primary doctor is sending me to Physical Therapy appt tomorrow. Am on MS Contin, Lyrica and oxycodone for breakthrough pain. Concerned about doing any PT to make this worse could this be cauda equina? Previous neuro told me after last fusion that I could expect Adjacent Segment Disorder and would need this one treated eventually. So these are my concerns what would your recommendations be follow thru with PT evaluation?Any further advise is appreciated, I have been thru alot and can take alot of pain, but its gone on so long now, its really wearing on me.

You are having degenerative changes and mild to moderate disc prolapse at mainly at L1-L2 level.You are likely to benefited by PT (physiotherapy). PT, analgesic and mecobalamin will be helpful.
