Posted on Tue, 28 Apr 2015
Question: Hello
I had vigorous sex 4 days ago and noticed pain in my lower abdomen upon orgasm. After that later at night my clit was very sensitive and painful next day I used over the counter yeast infection medication and till today no relief my clit felt very sensitive and a sort of burning pain when i urinate the pain is constant and burnign especially at the tip of the clit. I saw a dr today he check my urine and said that there was blood in it so he give me an Rx for antibiotics also near my vulva i have had a small mole very small and brown for a couple of years that also feels sensitive it has not changed in size since then my dr said that it did not look suspicious and to keep an eye on it that hasn't changed in size or texture its small like the size of a sesame seed. That mole feels sensitive too I am very worried i wonder if it was the vigorous rubbing during sex that caused the clit to become like this and then became infected it was more then usual. I am extremely anxious. The top of the clitorus is very painful and sensitive.

Brief Answer:
clitoral lacerations...

Detailed Answer:
Hello,
Thanks for the query to HCM,
It is evident from the history that you had vigorous or hard sex 4 days back and likely that you have caused injury to your clitoris in form of abrasions or mini-lacerations which causes the burning and pain while urination and often your urine sample XXXXXXX also be affected by red cells from clitoris during sample collection.

In absence of any infection, such abrasions are actually self healing , just that they have to be kept dry as often a s possible and local emollients or antiseptic ointment like SOFRAMYCIN ointment available as OTC can be applied 3-4 times a day especially post drying after each urination.
This shall prevent infection and allow early healing.
Avoid any kind of stimulation of clitoris, avoid intercourse till healing completes and wear loose under garments to prevent occasional frictions due to undergarments touching and freshening the abrasions. This usually takes 4-5 days to heal.
there is no harm to be started on empirical antibiotic cover, you can continue that for additional protection and assistance to healing process.
I hope you concerns have been addressed in detail,
Regards

Hello Dr Kuma

Thank you so much for getting back to me.

I am also having pain on the lower right side of my abdomen and it is radiating down to my leg. I am ovulating and i wonder if my husband and I have sex in two days would that be too soon we are trying to conceive and i had a miscarriage on Feb 27th and had a period 4 weeks after and now finally ready to try to conceive again and this happens. Do you think the pain on the right side is from this or something else also i look at the area and it doesn't show any lacerations but i guess I am not able to see them and they might be small. Is there any other home pain remedy that might work the ointment you mentioned I cannot seem to get at our pharmacy I am located in Canada is there another one that you know of.

Thank you

Detailed Answer:
Well if you have had miscarriage in last week of february then ideally you should be waiting for at least 3 months before trying to conceive again as uterus requires time to return back to its pre-pregnant state.

1. the pain in the lower abdomen can be due to ovulation itself and is known as MITTELSCHMERTZ which can last for 2-3 days.

2.Soframycin is FRAMYCETIN SULPHATE. you can ask your chemist for the salt based ointment. It may be manufactured under another brand name.
Just follow the instructions and it would be better if you plan from the next month for a child.
