Brief Answer:
Microangiopathic ischemic changes means ischemia due to small blood vessels

Detailed Answer:
Hello XXXXXXX

Microangiopathic ischemic changes in the white matter means ischemic( decreased blood supply and oxygenation due to involvement of small blood vessels)changes in white matter of brain.

Ischemic changes generally occurs in conditions like hypertension, altered lipid profile, diabetes mellitus, vasculitis etc. These conditions damage small blood vessels and this leads to ischemia.
So,you need screening of these conditions. It includes proper clinical examination and investigations like routine hemogram, random blood sugar, liver function test, renal function test, lipid profile.

Pan sinusitis means inflammation/infection involving all sinuses(including frontal, sphenoid, ethmoid and maxillary sinus). These sinuses are found in facial bones and it is generally associated with allergy. So,you should avoid allergen if you have known allergy.

You may need to take few medicines after evaluation.
You should also increase your physical activity and try to be active in daily activities.
