Question: My husband SGPT is 188, please help me to find out disease

Yes I've reviewed the attached investigations..

First of all, please let me know about his symptoms and the associated problems..

Since when everything has started ?

What was the initial problem and what all treatment has he received so far ?

I can see all these reports are of 24/08/16 which is 9 days old..can you please let know what exactly you meant by the urgency ? Is he at home/ hospital -getting treated ?

Is his physical condition deteriorated in this time span ? how is he doing presently ? Is there any fever/ pain abdomen/ loose motion/ vomiting/ breathing difficulty/ altered sensorium/ decreased urine out put etc. - which has developed recently ?

Is there any other recent investigations done ?

What is the current treatment he's receiving ?

As far as these previous investigations concerned, his CBC was grossly within normal range only, Malarial antigen was negative, WIDAL doesn't have a very specific value and in fact it's always reported in ratio(positive or negativity is decided by the treating doctors after correlating the patient's clinical findings)..

SGPT is definitely elevated significantly but it needs a complete Liver function test to assess further..please note that all these findings are to be clinically correlated(after examining your husband along side detailed history) by preferably an Internal Medicine specialist to come to a certain conclusion..

He is getting fever repeated in weeks time since last 15 days, these tests were done on doctors advice.

He is feeling tired very soon, loss of appetite , Vomit due to cough due to weather change at XXXXXXX MP.

I can see some yellow XXXXXXX in his face, eyes & nails.

Not yet hospitalized, taking medicines at home with doctors advice.

Thanks for following up,

The fever along with these associated symptoms have to be evaluated in details, if you mean till now, only this much has done so far, then definitely it needs detailed investigations and a proper clinical examination..

You haven't mentioned any thing about the current treatment he's receiving (please don't rush, and feel free to share the details in order, so that I can guide you more specifically)..please mention the exact names of the medicines he's receiving at present..

All those symptoms (feeling tired very soon, loss of appetite , Vomit due to cough etc.) are mainly due to tropical illness (yes, rainy season is one of the culprit of this), there's a whole list of them, so the exact cause has to be found out after thorough clinical examination, investigation along with a proper detailed history..also try to look for any rash/ bite mark over any body part (rainy season/weather change is another culprit for vector/tick born infections)..

He needs a through check-up by an Internal Medicine / Infectious Disease specialist (whomever is accessible by you at first) without any further delay..followed by a battery of relevant investigations (repeat CBC, peripheral blood smear, complete LFT, KFT, CRP, Blood-2 site culture sensitivity and urine culture and sensitivity, if any loose motion then stool routine microscopy along with culture and sensitivity along with Typhie Dot IgM, Dengue serology, Hepatitis A & E IgM and IgE , Chest X-ray, Ultra-sonography of abdomen etc.)..looks a lot of investigations but they are required in order to get it properly diagnosed..

The yellowish discoloration is mainly due to hepatitis, which is a part of the illness only, so needs proper evaluation without any further delay..already 15 days have passed by, please seek out for an Internal Medicine Specialist or an Infectious Disease specialist (you can find them in your nearest multi-speciality hospital's OPD right away, if you can't find at a glance, simply google it) in order to get him appropriately treated..

Meanwhile, ensure proper hydration (drink only mineral/ U.V.R.O purified water only) of ~ 2.5-3 ltr per day..
