Question: I HAD A CT SCAN OD MY SINUSES AND THE DR. SAID THE IMPRESSION WAS: 1. PREVIOUS SURGERY TO THE SELLA TURCICA SPHENOID SINUS. OTHERWISE NEGATIVE CT OF THE NECK. WHAT DOES THIS MEAN?

Thank you Dr. Morati I appreciate yur honest answer. I went back and checked my records and come to find out you are absolutely right. I had a Transnasal Transphenoidal resection of the Pituitary Adenoma. They found a tumor present in the left inferior sella. When they got it they found some abnormal tissue consistent with tumor in the bone. They found no remaining pituitary gland. However, I am very sick right now and am having some of the same symptoms I was having back then. I am thinking that something is going on in my brain again and not my sinuses or GERD. I am going to contact my Dr. in XXXXXXX and tell him what is going on.

My ENT suspects that the drainage in my nose and throat are due to CSF leak. I had that when I had my Brain Tumor removed through the nose. So may be it could be?

You may need an examination of this fluid on the back of your throat to see if it has same characteristic of CFS.

Will do. I know that when i was in surgery the dr.s had to stop surgery for awhile because i had CSF Leak. They also say it may not be a cf leak but a remnant fluid fro the packing of fat in sphenoid sinus. I ill let you know what the dr.s say in XXXXXXX my dear. Thanks for caring.

I have got you saved in my contact list so that I can contact you when I gt back from XXXXXXX It's going to be a long and drawn out ordeal. It always is. The longest time I have to wait is in between Dr. visits on the 26th. My first appointment with the Head of Neurology is at 8:00 am then my next appointment to see my Endocrinologist is going to be at 3:00 pm. That's a big time difference. If I didn't have to wait on him I would already be at home by then.

If your doctors in XXXXXXX suspect that you might have CSF leak ,there are a number of tests they can do to confirm it and to localize it.
-First they can examine the nasal fluid for a protein called beta-2 transferrin which is most only found in cerebrospinal fluid.In this way they can know if the fluid is CSF or just nasal secretions.
-They can do a nasal endoscopy to examine the inside of nose .
-Ct-scan
- A coronal CT cisternogram which involves using a contrast injected into the spinal fluid through a spinal tap and then performing CT scans. This test identifies the exact location of the CSF leak and the pathway of the drainage into the nose.

Your doctors will chose the best approach for this problem.

Yes my dear. This was a big help to me. I now know what tests can be done to check for the CSF leak. I just wanted to let you know that I have already had Nasalendoscopy done to check the sinuses.It came back good. They found no growths or polyps, etc. I had a ct of the nasal cavities as well and they came back clear. No sign of infection as of July 25. It was clear. So i am clean as of right now for a sinus infection. That's why Dr.s are leaning towards a CSF Leak. They did a CT scan prior to this one in the beginning stages a few years back and they saw where I had an infection in the right side of my nose where I had previously had the transphenoidal resection of the Pituitary Adenoma. It had at times come out wtih green crust and cyst lik. Sometimes it would be a little bloody. I had to really watch it for a few days and if it got worse I was to go into the ER right away. That was a definite no-no. i was rushed to er in XXXXXXX 3 hours away right after the surgery in 2005 because of gushing blood out of my nose. They admited me right away and kept me for a week to check for a real CSF Leak. It's been real frustrating all these years jus going through all these mnedical issues. In the point you are now,after having done nasal endoscopy,and CT scan ,it is important first to examine the nasal liquid and to know if it is CSF or no.
If it is CSF ,you may need another high resolution CT scan and other investigations to localize the leak.

It is very important to exclude the possibility of CSF leak because it can lead to serious infection of meninges (meningitis ) and the brain.

Here we go: Why is it when I gargle with Listerine or any other mouth wash for that matter that my mouth and throat produce a massive amount of bubbles. They come out of my mouth like I am blowing bubbles like the bubbles kids play with? Also why is it when i am through gargling and rinsing my mouth out why does the it come out in a big, big pool of slimy spit? Truthfully...it looks and feels like a big slug? You know what a Slug is right? A Snail with a shell or big worm. Why is that? Where does the slimy spit come from? From my throat or where? Can you shed some light on this subject? That would be most beneficial to me if you could. That my friend would open a lot of doors and answers a lot of questions I have. It's even hard for me to drink water for that matter because it comes back up in a pool of spit right away. It doesn't have time to reach my stomach but it always seems to get stuck in the middle of my throat while i am swallowing. The water comes back up in a spray of clear spit. Any answers you can give me to shed some light on this subject? Please answer if you know why or have any clues as to why you can give me. II don't mean to be invading your space by asking you a lot of questions like I do but I have put my faith and trust in you as a woman and a Dr. who has the knowledge and experience to have an answer. This problem you are having might be related to excessive production of secretions in your throat.
Because Listerine and other mouthwashes contain alcohol they can dissolve the thick secretion in the back of your throat and it makes easy for them to get out.
Now these excessive secretions in the back of your throat may be due to some conditions like:
-GERD.In this condition the stomach acid moves up to esophagus and throat causing their inflammation and excessive production of secretions that may came out through cough.
-Secretions from nose (like allergy ,or CSF ) are drained in the back of the throat causing irritation of the throat ,cough and a lump feeling in the throat.

In my opinion since your ENT doctor has excluded other causes for this excessive secretions in your throat ,it might be related with the possibility of CSF leak.
Everything will be more clear after your consults and investigations in XXXXXXX to confirm or exclude CSF leak as an option.

How do I approach my doctor and tell and suggest to him that I think it may be related to a CSF Leak? He is going to to ask me why I think it is a CSF Leak. Thank you very much for your quick response to this question. You are great.
I will put one foot forward and go forward with this advice. The team of doctors at MD XXXXXXX include: Neurologist, Endocrinologist, MRI Team, and Blood work. The first 2 I mentioned are the specific consultants that I must meet with in order to get the reading of the MRI and go over the results of the blood workup tests. This is going to be a long 2 day ordeal. It always is. I have to learn how to be more patient though. I seem to get a little impatient due to the long wait in waiting room before your able to see your Dr. and the other one is the long wait in between doctor appointments. My first appointment is at 8:30 am and the other is at 3:00 pm that afternoon. That is a log time to have to wait in between appointments. Whew!!! I tend to get a little frustrated with this. It happens all the time when we go up to XXXXXXX for my scheduled follow-up appointments. This is every 6 months. That is not including my Dr. Appointments I have here at home. I just got to get used to it. It's going to be for life.

You are absolutey right my dear. I have to be more patient. That's the truth.
Another long wait is when I am going to have the MRI done. I have to get prepped first of all. They have to establish me with a line in my arm for the contrast to go in and then after I am prepped it usually takes a good 2 hours before they call be back to actually do the MRI. the MRI takes about an hour. I have to absolutely still or the tests have to be redone and rescanned again. I can't move a muscle or even make a little twitch or they will stop the scan and I will have to do that particular segment over again. i have to lay there absolutely still the entire time. No movement at all whatsoever. Now you know why it gets frustrating at times. Besides that I have no good veins. I have had my blood taken so many times that my veins have since collapsed. I am a hard stick. They usually find a vein in the upper hand region. Anyway...that's just the beginning.
I lwill let you know what the Dr.s say in XXXXXXX That's for sure. I promise.

I just wnted to tell you what happened to me this morning. I woke up and i have yellow marks with bumps popped up on my arms. You can feel the ridges that protude. Also my body is coming up with all these brown looking freckles but bigger. You also see more redness as if the flow of blood is showing up. t has a red tint to it. These things are not normal. What in Gods name is going on? I sure don't have a clue. But there is definitely something going on inside my body. What else is going to happen. My Lord when it rains it poors.
