What Do These Following CT Scan Findings Indicate?

Question: I'm writing on behalf of my mother, XXXX, who is 72 years old. My mother recently went to a doctor and here is the doctor's report below that she received. I am asking what do you recommend/prognosis at this point. The doctor said everything is fine except a few nodules on lung and thyroid. Doctor did order ultrasound on thyroid. My mother did have an ultrasound and another blood test regarding thyroid yesterday and within two hours, the doctor called and said everything is fine with the nodule on her thyroid. It wasn't until we read her medical report yesterday about the nodules/tumors/adenoma and the kidneys.I don't feel she is given adequate medical care/advice. Based on my mother's doctor's report, I'm asking for a second opinion. What do you recommend my mother do, besides additional testing and seeking another doctor?

Lungs and large airways: Moderate emphysematous changes of the lungs.
Groundglass pleural-based nodule in the right upper lobe image 49
series 4 measuring 5 mm. Linear scarring or atelectasis in the
bilateral right greater than left lung bases. 3 mm nodule in the right
upper lobe image 39 series 4. 4 mm nodule in the left lower lobe image
67 series 4. 3 mm nodule in the left lower lobe image 43 series 4. 2
mm nodule left upper lobe image 33 series 4.

Pleura: No pleural effusion or thickening.
Heart and pericardium: Heart size is normal. No pericardial effusion.
Mediastinum and hila: Nonspecific nonenlarged mediastinal lymph nodes.
Chest wall and lower neck: Heterogeneous nodule in the left lobe of
the thyroid gland for which dedicated thyroid ultrasound is
recommended..
Vessels: Atherosclerotic changes in the aorta and coronary arteries.
Bones: Mild degenerative changes of the spine.
Visualized upper abdomen: Visualized upper abdomen demonstrates
nodular appearance of the bilateral adrenal glands which demonstrate
Hounsfield units of a lipid rich adenomas. Left renal cortical
hypodensity likely represents a cyst measuring 13 mm. Left 3 mm
nonobstructing calculus partially imaged on this exam.

IMPRESSION:
-Moderate emphysematous changes of the lungs
- Bilateral lung nodules measuring up to 5 mm. Short interval
follow-up with CT chest in 6 months is recommended for further
evaluation.- Heterogeneous nodule in the left lobe of the thyroid
gland for which dedicated thyroid ultrasound is recommended
- Nodular appearance of the bilateral adrenal glands which demonstrate
Hounsfield units of a lipid rich adenomas.
- Left renal cortical hypodensity likely represents a cyst measuring
13 mm.
- Left 3 mm nonobstructing renal calculus partially imaged on this
exam.

I have gone through the medical report of your mother in details and will try to explain step by step each of the problems found in the CT-scan.

1. Heterogenous nodules of thyroid. Ultrasound of the thyroid is one of the best exams to do, but if I're the caring doctor of your mother i would recommend to do the blood tests , thyroid hormones test ( Ft3, Ft4,TSH). If the hormones are within normal range then there is nothing to be worried. If there is a problem with these results than (as a surgeon) a fine needle aspiration study (FNAC) might be the best option to determine if the node is benign or malignant.

2. Changes in the lungs are a normal phenomena at her age-group, especially if she smokes. The nodules of 5mm in the lungs found in the CT might be from a lot of reasons. So i'd agree with the doctor that she needs to do another CT-scan 6 months later.

3. Lipid rich adenomas in adrenal glands....nothing to be worried. The percentage to be malignant are minimal.

4. Cortical renal cyst ...nothing to be worried. It is a normal finding and the stone- if symptomatic then only needs further evaluation.

I'd suggest: -- blood tests for thyroid hormones. Depending on the results, it'll be decided whether she might need any further exams or treatment or not.

-- Repeat CT-scan after 6 months
