Bilateral hypertrophied temporalis muscle.

I have gone through the reports uploaded. MRI is suggestive of bilateral temporalis muscle hypertrophy, other than that there is no intracranial damage.

Temporalis muscle hypertrophy can be reactive to certain jaw conditions such as bruxism. A dental evaluation may be helpful.
Further, diseases like Fibromyalgia, myositis, inflammatory myopathies, dystrophies can present with temporalis muscle hypertrophy.

Temporalis muscle hypertrophy can cause chronic headache.

Harmful or not, depends on the cause.If its reactive due to some jaw issues, its not harmful.If its due to some secondary conditions as I mentioned above, the prognosis depends on the actual cause.

Symptoms can be chronic headache, fatigue and giddiness.

Why does it happen-the causes I mentioned in the above discussion.

We need to address the cause. If its reactive, we should correct the jaw issues. If its some secondary disease, then medicines depends on the disease.

Basically, we need to investigate for the cause first. A dental evaluation and a neurologist's in person consultation will help.
