Question: I suspect that I may have some form of myositis. what are the symptoms of myositis and how is it diagnosed? what tests need to be done before a diagnosis can be made? How common is this disease? I ask because I 've had high cpk numbers since the spring ( about 6 months which have fluctuated from a low of 600 to a high of 1200.

Muscle biopsy would be required if infectious myositis or inclusion body myositis is suspected. In dermatomyositis , skin and muscle both biopsies may be advised to confirm or rule out the diagnosis.

The symptoms include muscular pain and myalgia. ANA profile, LDH, ESR/ CRP levels can be advised. Imaging with ultrasound of the affected muscle group can also be used adjunctively to rule out any collection.

Statin use can also lead to elevated levels of CPK. If statin use history is present and its further indicated then ezetimibe or other option as an alternative can be utilized based on the lipid profile or respective indication.
