Question: My wife getting a result in FNAC test as she is affected with granulomatous mastitis. What's the treatment for this case?

Read both the reports and understood the concerns.
My opinion with reasoning wherever needed:
- Since left breast is showing 2 different lesion, one is fibroadenoma and another is as per the FNAC also a granulomatous Mastitis - Birads IV, I would strongly advise her to get the formal excision biopsy for 2 reasons:
1) She gets rid of the disease, which is not a normal part of the body.
2) You get an exact diagnosis by Histo-pathology examination, a part of the granulomatous mass can be sent for GenExpert for TB. Since both the lesions are small a proper surgery will not disturb the breast architecture.
It is the final report of histo-pathology that will decide the nature of treatment.
I hope this answer clears the method of proper diagnosis and proper treatment.
Anything other than Excision biopsy is only going to give an idea about what can the disease be but can not tell for sure what exactly is the problem. Hence anything like mammography, MRI can be skipped to get a definite diagnosis.
