What Causes Urge To Urinate After Anterior Repair And Hysterectomy?

Question: I had an anterior repair and hysterectomy 1st October last year.

Since the surgery I have felt the need to pass urine, I had an ultra sound 3 months ago which showed a residual urine of 150mls which was repeated two weeks ago which showed 50mls which I was told was normal.

I have also had a feeling of "something coming down" for the last few weeks. I saw the gynaecologist this week who said there was no problem with the surgery.

Today I have done some exercise and the feeling worsened. I have had a look at myself and on straining and can see bulging at the entrance to the vagina. Also as I touched this the need to pass urine increased.

I'm confused as to what this is, and only a few days ago I was told all was well. I feel these are the symptoms I had before and I have had this surgery. This has upset me this evening hence this email

Vault prolapse is where the uterus has been removed?
I saw him a week ago today and he said (I can't remember the word he used) but I took it to mean this area was high up.
I had felt a dropping sensation when I saw him but cannot understand why he said there was no problem. He actually has discharged me.
I'm upset as after going through the surgery I feel I'm no better off than I was, although I did have a big cystocele.
I was hoping it was the walls of the vagina I was seeing but I do not feel it is this. Is that a possibility?
You say do not do any lifting which I will take your advice but what about running?
I'm worried that he won't see what I am describing. Is a prolapse hard to diagnose if it is not at it's worst?
What would now be the treatment?
I feel now that I wish I had not had the operation.

Vault is the area from where the uterus has been removed. If it is high up then there can be a cystocele and rectocele. You can continue running.
A careful examination will help in detecting if there is any prolapse. You need to describe about your problem in detail. Using a sling can help you.
The bulge that you saw could be the vaginal wall also. Just consult your surgeon and get a repeat examination done for confirmation.

OK, so it is not a vault prolapse as he said this was high. I asked if I had a rectocele and he said no. The vaginal wall would feel softer? If it was the wall would that cause the urinary symptoms? What I feel is harder, although obviously I don't know what I'm feeling. This lump seems to be more posterior.
I only had surgery 6 months ago. Would this have deteriorated again so soon? Is this common? Is it possible the surgery was not done to a good standard? I'm not blaming but want to know facts. Is it advisable to have further surgery so quickly.
I have described my symptoms in detail to him, especially the continual feeling of wanting to urinate. I think he feels I'm being neurotic but now I have seen the "prolapse" my eyes are not deceiving me.

A firmer lump posteriorly is not likely to cause the urinary symptoms. It is possible that you are pressing on the vaginal wall itself which is a normal finding.
The deterioration is not expected just 6 months after the surgery unless you did something to put extra pressure on the area. The quality of surgery can be assessed only after a direct examination. You can take a second opinion for this.
In case you have symptoms and discomfort then repeat surgery may be needed. Local estrogen cream application can also help.
Vaginal wall is more firm than the prolapsed bladder wall. Your surgeon will be able to differentiate between these two. The frequency of urination can be due to a urinary infection also. Do get your routine urine examination done.

Yes I realise I need an examination. I appreciate the answer to my questions so I know the right questions to ask.
The "lump" looks as if it is coming from inside, like it did before but I do so hope it is the vaginal wall. If it is the vaginal wall would it be worse with straining? This would not explain the urinary symptoms then?
No, I have not got a urine infection and I have not done anything to cause damage, in fact I have been more cautious than he said. He said I can lift etc. I have got some oestrogen pessaries.
I didn't mention this was all done vaginally.

Yes. If it is a lump in the vaginal wall then it does not explain the urinary symptoms. It should not become worse with straining.
If you have some estrogen pessaries then you can use it and see if it helps to decrease your symptoms.

Then it doesn't sound like the vaginal wall if that would not give me urinary symptoms. How can I not worry when I have spent a lot of money and been through an operation to then feel it has not been successful.
I can't really understand how the oestrogen helps. He did say there is a lot of atrophy so can see how it can help that, but how can it help the urinary symptoms and "lump"?
