Question: Hi I have been diagnosed as having hypothyroidism back in 1992 and am now up to 200mcg of levo.Yesterday I developed a low grade fever . I thought I was getting a sinus infection because I always get them when the weather changes. I had some soares on the right front side of my neck. This morning my front right side of my neck is very swollen and sore to touch. Also my testes are sore . What should I do? I have never had this issue with my neck but have had the testicle soreness when I have had a sinus infection.

Brief Answer:
You need treatment as suggested!

Detailed Answer:
Thanks for consulting me at virtual clinic. Your trust in HCM means a lot to me as a Physician.
I have completely reviewed your health query, and can well realize all those concerns. Being your physician, I will take care all of them today and will give extend my expertise to solve the problems.

Usually testicles get sore with this type of neck involvement in mumps. But as your history shows, you always get soreness of testicles alongwith sinus infection. So, it goes more towards being Acute Pharyngitis (infection of the throat and sinuses may also be involved).

Neck swelling in such infections is very common due to lymph nodal involvement. So, it will get back to normal once the infection is over.

Management guide:-
Prophylactic antibiotics are necessary and if I was your treating Physician I would have prescribed you with Azithromycin if not allergic to. Plus, Fexofenadine by mouth. You are already taking analgesics so no need for additional ones. Do gargles with saline lukewarm water. Keep a judicious supply of clean drinking water. Keep balanced and healthy diet. Rest is needed for fast recovery. Avoid dust and animal dander.

Hope I answered all of your queries. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with my expert opinion. Yes Doctor visit will be beneficial as he can palpate and diagnose it properly. Ask him to rule out Mumps too. Lymphadenopathy and any other possibility should also be taken into account like an Ultrasound of the thyroid gland to see the status.
