I appreciate your concern

If this was the first episode of such a headache and it was severe then the ER doctors were most likely worried about a possible subarachnoid hemorrhage which has been ruled out thanks to the CT scan. The CT scan can easily pick up a subarachnoid bleed and is more sensitive than an MRI with respect to Subarachnoid hemorrhage. Your doctor in the ER would also have examined you for possible focal neurological deficits. Most probably no serious abnormality was found due to which they referred you back to your family physician.

Kindly answer the following to advise further.

Have you had this sort/type of headache before
Is it always associated with nausea or vomiting
State your Blood pressure.
By pressure on your bowels, which area of the abdomen do you refer to specifically.
Has the buzzing in the ears subsided or is it still there.

No history of headaches, much less of one with this much pain. Blood pressure one hour later was 158/74. Incident recurred today starting with an urgent need to evacuate bowel, severe pulsating headache with buzzing in ears.

Hello again.

Ibuprofen 600 mg alone or 400 mg with paracetamol 500 can be taken for now. If the headache does not improve within an hour then metoclopramide can be advised in addition. Sinusitis should be looked for on physical exam and if needed an x ray PNS can be done to look for nasal septum deviation, opacification of sinuses etc. Antibiotics and decongestants would be the main stay of treatment. Brain Aneurysms can present with sudden onset of a severe headache. However, the ER assessment did not find any neurological abnormality and if there is no history of polycystic kidney disease or connective tissue disorders which are associated with berry aneurysm of brain then an aneurysm is less likely. If your doctor still suspects a sinister diagnosis like aneurysm then a CTA or MRA can be advised.
