Question: Hi, me and my husband earlier were on contraception and stopped the contraceptive mode (condoms) over two weeks back and over the past one week plus, I have been noticing a kind of bloating sensation in my stomach, feeling fatigued, having no appetite, wanting to eat, but the smell of food makes me not feel like eating at all and if I have it having a d; can u please help me on this, as to what the exact cause might be, if I need to consult a GYN or a family physician regarding this.

I have gone through your concern. Are you concerned about the probable pregnancy?

From your history, few symptoms like loss of appetite, nausea, bloating sensation in the stomach may be suggesting pregnancy. However, these symptoms point towards pregnancy only if you have missed your menstrual period. Will you please tell me details about your menstrual history and the date when you had last menstrual period? Do you have other pregnancy symptoms like frequency of urination and breast discomfort in the form of engorgement, tenderness and nipple secretions? Have your nipples and areola become darker in colour and has the areolar size widened?

You have discontinued use of condom. May I know why? Are you planning to go for pregnancy or you have shifted to some other contraceptive method? This information will tell me your intentions.

At present, all the steps you have to take will depend upon your last menstrual period. If you have missed your period, please get your urine pregnancy test (UPT) and/or ultrasonography performed 8-10 days after the missed period for detection of pregnancy. These tests done earlier may give false negative or inconclusive results.

If you have not missed period as yet, these symptoms appear grossly due to gastric upset - mostly increased stomach acidity or indigestion for which you might seek the advice of your family doctor.

The last menstrual period was on 22nd of last month. I do have frequency of urination. I didnt pay attention towards any other changes, as I was not aware regarding any as such. We have discontinued the contraceptive use as we were planning for a pregnancy.

I have as well used oral contraceptives for one month before (nearly 2 months prior). Wanted to know as well regarding use of any oral contraceptives wud have any lethal effects on the kid in case of pregnancy?? as I heard thru someone that the couple shud plan for a pregnancy after 6 months of stopping oral contraceptives,else it wud be lethal for the fetus and it may result in complications either physically or mentally. Please detail me regarding all the possible options that can result due to this.
I do have severe backache with the bloating sensation and a sense of feeling tight in the abdomen with the rest of the symptoms as detailed above.

Dear XXXXX,

Thanks for the information on some questions that I had raised, though not all.

I presume your last menses (LMP) to be on 22nd XXXXXXX since you have not mentioned the month. If it is 22nd July, the question of pregnancy does not arise at all since you would not have missed a period.

If your LMP was on 22nd XXXXXXX now it is more than 6 weeks of missed period; so if you perform urine pregnancy test and/or ultrasonography, you will have the conclusive results for detection of pregnancy. Also please pay attention to the other symptoms of pregnancy, particularly the breast symptoms which by now must have started appearing.

During first three months, usually there is no reason for low back ache. Please get blood examined for evidence of anaemia. However, even if the haemoglobin levels are low, I would advise you to start iron preparation only after 3 months of pregnancy since earlier, there is nausea and vomiting of pregnancy which may get worsened by iron. Also start calcium preparation daily which will give you relief from low back ache. Particularly XXXXXXX ladies are known to have calcium deficiency.

And do not forget to start Folic acid with Vitamin B12. Folic acid deficiencies are known to cause abnormalities in the baby.

Now your query regarding the effects of contraceptive pills on the baby. It is generally advised to attempt for the pregnancy six months after discontinuation of the oral contraceptive pills because there is an increased risk of pregnancy getting terminated spontaneously for pregnancies within six months of discontinuation of contraceptive pills; however, it is not a rule. Large number of women take such pregnancies to the term without any ill effect on the baby. If you have become pregnant, it is not an indication to terminate the pregnancy during this period. If the pregnancy continues, which in most of the cases happens, the risk is gone.

Still if you are apprehensive about the effect on the baby, you may request your Obstetrician for performing certain tests like Triple marker test, nuchal translucency test which point towards some foetal abnormalities. Though chorionic villus biopsy can diagnose all the chromosomal abnormalities, I do not think pregnancy within 6 months after discontinuation of oral contraceptives in an indication for this test as it is quite an invasive test.

Now a days, triple maker test and the nuchal translucency test are offered to almost all the pregnant women during first trimester. So you also may be offered them.

You may discuss your concern with your Obstetrician and you will get the same explanation that I have provided you.

I hope your pregnancy tests are positive and you take the pregnancy to the term to deliver a healthy bonny baby.
