Question: Hi, i am a 34 year old woman suffering from PCOD and irregular periods for years. My doctor had recommended 6-7 cycles of Diane 35 this year to regulate the periods. After 7 months on this pill, I stopped taking it this month. I got the period as usual after stopping the pill. However, after a week of the period ending, I have again started getting a light period bleeding. Is this because of stopping Diane 35? Is this a normal reaction and should I take further medicines now?

Thanks for the query to HCM,
This is known as breakthrough bleeding and can often be seen for 1-2 cycles after prolong usage of oc pills like diane 35 even for PCOS.
Cos is a collective disease where not just testosterone levels are high and have to be controlled but also insulin resistance would have to be decreased by losing weight. If either one is lagging behind, anovulation due to PCOS is often not cured. SO present breakthrough bleeding can be controlled through antifibrinolytics like Tranexamic acid tab 500mg twice a day for next 3 days. you may get it prescribed by your gynecologist.

Hi doctor, thanks for your reply. I will try to lose weight to control pcos. My periods were very regular in these months when I was using the pill. Prior to that I was getting periods every 3-4 months only on taking regesterone 5mg. Is regesterone tablet safe to use repeatedly if my periods again stop now after stopping Diane 35?

Also, should I start the tranexamic acid tablet right away or can the breakthrough bleeding stop on its own in few days? Can I continue to use this tablet each month if my periods start coming back within a week like this each month now?

I do not want to continue using Diane 35 any more for my PCOS, hence asking these questions because I'm not sure whether I will start getting extended periods like this or no periods from now on.

Hello,
As told earlier, weight reduction is a must in PCOS, ELSE DIANE SHALL NOT HELP. So has happened in your case. ideally you should give a gap of 2 months now and if menses become irregular again after being regular on diane35, then you have to be started again on diane35 along with insulin sensitisers and weight loss regime with exercise and diet.
You can start tranexamic acid right away as that shall help control bleeding in next 6 hrs. Dose is 500mg twice a day for 3 days. It can be used if the bleeding is heavy only, else for spotting its not really recommended.
Please maintain a menstrual calendar for next 3 months and analyse the pattern of menses. If the pattern is irregular again then diane 35 might have to be restarted.
PCOS is like diabetes, it can be controlled but not cured.
