Question: I am 28 year old married female with Mirena for the past 4 years and 7 months. Never had issues with it-no real periods just occasional spotting. I have had numerous yeast infections in the last few years (my sisters get these often too). I know symptoms so i have Diflucan Rx so when I feel one I can take a pill. Saturday early AM I woke up with a little bleeding but my vagina was super sore/red inside. I have not had sex recently and have not had any new sexual partners in 5 years. I took Diflucan immediately. I took another 24 hours later and felt somewhat better but now have pain if I touch vagina. I have no itching or burning (which is a normal yeast infection symptom for me). I have no urgency to urinate more frequently. I spent a week at the lake last week in a swimsuit that was wet many times. I know it's early on but could this be just a yeast infection with slightly different symptoms (i.e. no itching)? I don't have much discharge, just a tiny and it's white/slightly lumpy. But very little. Do I need to consult doctor or should I just wait it out a week for Diflucan to work? I have a little low pelvic pain but mainly it's just the vagina that is sore to touch. No issues if I'm just sitting or walking. Please help!

This sore vagina is most probably yeast infection again and complicated this time because of either that swimming event or may be because drug resistance because of repeated episodes and same drug every time.
In my opinion you need to visit your gynecologist for examination so that she can see if this is yeast or something else.
Also you need to get certain tests done like complete blood count and fasting blood sugar levels.
In future use condoms every time during sex till you are not relieved of symptoms.

If it was something else, what could it be? It is red and sore on the inside. If I touch it it almost feels like someone stabbing me directly in the vagina. I know that sounds odd. Could it just be that the medicine hasn't had enough time to work (normally doctor makes me allow 7 days)? I don't have any odd smells that I've noticed, no burning while urinating. If I insert my finger directly into my vagina it's almost like it's swollen. I know you said yeast infection (and that seems to be my track record) but I've just never had pain inside my vagina like this. For lack of a better explanation it's almost the feeling when you have rough sex and it hurts the next day. Again - haven't had sex recently, one sexual partner for years and he hasn't been with anyone else ever. Sorry to keep bothering you!

Its a pleasure to answer your query mam because its a rarity to have such well informed patients.
In my opinion its a yeast infection which got worse because of the swimming event.
But swelling inside vagina is not a good sign so you need to add a local clotrimazole cream or pessary so as to cover the infection this time.
I usually prescribe clotrimazole vaginal cream to be used twice a day for a week along with fluconazole tablet.
Along with these drink lots of water and keep hygiene of your private parts and get the tests done which I have prescribed you.
Repeated infections with yeast can be an early sign of diabetes or poor general health ad in anemia.
After the tests we can discuss the reports anytime.
