What Causes Severe Shifting Backache?

Question: Hi Doctor,
Iam having severe backache since 3 days. On the first day the pain was very severe that i could not even get up from bed. The second day the pain diminished but the still there is a lot of pain on the lower back esp on the left lower part when i get up from bed or when i turn toward one side in the bed. Third day also i feel the same as second day. Also, When i lie down resting on back, there is no pain, but when i sneeze i still feel pain. I saw a doctor, and when he made me lift the LEFT leg(lying on bed) leg upwards i could feel the pain in the lower left side of my spine. He suggested a x-ray(all attached here).From X ray he told me nothing to worry about and i had a curve in spine which is there since birth and since iam obese, it would be more visible than ever and hence told me to reduced the weight. I wanted to know your opinion about this? Is this serious? Iam quite concerned.

Also, do you see any disc problem from the X-ray?

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I have gone through your elaborate clinical history and X-Ray Lumbo-sacral spine.X-Ray shows scoliosis in lumbar spine.It can be present since early childhood or a result of obesity.However,it is not the likely cause of your symptoms.Nor,it is possible to comment precisely on the status of discs whether they are normal or not.

At this moment,there are two approaches to deal with it:
1)Start doing spinal extension exercises under the guidance of an expert physiotherapist.Simultaneously,analgesics like non steroidal anti-inflammatory drugs can be taken for a few days, as etoricoxib or naproxen.With efficient and regular exercise,these symptoms should improve.If no improvement occurs in a couple of weeks,Magnetic resonance imaging(MRI) of lumbo-sacral spine should be done,to see the discs and roots.
2)Alternatively,its better to get the MRI done from the beginning and to know the status of roots and discs.The etiology can be then,managed accordingly.

Besides,If possible please consult a Rheumatologist for the possibility of ruling out a hip joint or sacro-iliac joint pathology,which can sometimes present like this.This possibility,though,is down the list here as against a spinal etiology.

Thanks for being in follow-up.

I am afraid to say that degree of curve can't be appropriately estimated on X-Ray.MRI will be helpful for this.I can however say that scoliosis is moderate.
