Brief Answer:
Check blood pressure and heart rate

Detailed Answer:
Dear Sir,

I would advise you to monitor your blood pressure and pulse. Measure them three times a day and especially before going to bed, when you have complaints.
Such symptoms are often caused by blood pressure and/or pulse variations, usually in case of high blood pressure and/or fast heart rate. And symptoms usually disappear with the treatment and blood pressure and heart rate regulation.
If there is no alteration in blood pressure or heart rate, I would advise you to repeat the carotid ultrasound.

Hope I could help you

Wishing you good health

Brief Answer:
Welcome back

Detailed Answer:
Dear Sir,

Most frequently patients hear their heartbeat in their ear, when they have not controlled blood pressure or pulse. Usually they hear it at night or in the evening, as in this time noise from the surrounding environment is less.
In your case it could be related to high blood pressure. See your doctor to prescribe a treatment to regulate blood pressure and your symptoms will disappear gradually.

Take care
